Medical emergency, how would it affect transfer application process?

Hi,

How would Medical Emergency look like on transfer application?
I might need to do surgery.
I was given 2 choices:
1) Take the finals anyway, might not do well, might do well (Chance to raise GPA but I'm kind of behind now and not sure how much I can catch up), so maybe explain what happened to me in the personal statement. (But I'm worried it would look like "sick person" and they would not like to take students like this...)
2) take finals later with pass/fail grade. (no influence on GPA but would not raise it neither...my first semester was not that great, just average). it might be letter grades, but not guaranteed.

I'm not sure what to do...
Would appreciate any ideas.

Well if you have just average grades, why do you think you will be able to transfer?

I thought I can do better on the 2nd semester bc I found where I did wrong last semester.
Also I know some of the profs in the school who want me to come to their school, and I want to assist their work too...
